Complete your self-reporting survey by providing a few bits of data: Name, City, State, Zip Code, Gender at birth, race/ethnicity, and Sickle Cell Status (SCD, SCT or unsure).

Verify Your Email
Provide your email address to Wanda and verify it to activate your account and select your free gift card, Uber Health, MyID Medical Alert, and virtual training benefits.
Registration benefits come in three different bundle options for those with SCD and one bundle for those without.
